Collaboration of Many Minds, Healthy Food and Public Health
Summary
Poverty, food deserts, lack of education and access to health care lead to poor health outcomes. Working with the established 12th St Health and Wellness Center (HWC), the Division of Culinary Medicine and American Heart Association, we propose to provide lifestyle counseling as well as provide nutritious foods through food pantry/cooking demos/providing hot meals/assist with SNAP and Medicaid applications. Additional partnerships include the Community Advisory Board of 12th St HWC, Trustees of St. Andrew’s Methodist Church on Baseline Rd and AR Food Bank.
Serial blood pressure, Hba1c and step tracker measurements will be utilized to measure health outcomes associated with healthy food choices.
Student volunteers, under the leadership of preceptors from 12th St and Culinary Medicine including Registered Dieticians will convene once monthly at the 12th St Station and once monthly at St. Andrew’s Methodist Church to provide healthy choice food items, cooking demonstrations. Patients will be counseled on lifestyle activities. We hope to provide patients with BP cuff for home monitoring and a step tracker and reassess at 3 and 6 months.
